About the Position
Grand Home Health Services is seeking experienced Registered Nurses (RN) to provide skilled nursing services and clinical oversight for clients receiving home care services.
The RN is responsible for developing care plans, supervising nursing staff, and ensuring high-quality patient care in home-based environments.
Position Responsibilities
• Conduct patient assessments and health evaluations
• Develop and implement individualized care plans
• Provide skilled nursing care and treatments as required
• Supervise and support LPNs and home care staff
• Monitor patient progress and adjust care plans when needed
• Provide health education and guidance to clients and families
• Ensure compliance with home care regulations and agency policies
• Maintain accurate documentation of all nursing services provided
• Report and respond to changes in patient health status
• Maintain compliance with HIPAA and patient confidentiality standards
Minimum Qualifications
• Current Pennsylvania Registered Nurse (RN) License
• Graduate of an accredited nursing program
• CPR / BLS certification preferred
• Home care or community health experience preferred
Required Pennsylvania Clearances
• Pennsylvania State Police Criminal Record Check (Act 34)
• Pennsylvania Child Abuse History Clearance (Act 33)
• FBI Fingerprint Clearance (Act 114) when required
• Act 168 Employment History Review when applicable
• TB screening if required
Required Training
• HIPAA Privacy and Confidentiality Training
• Infection Control Training
• CPR / Basic Life Support (BLS)
• Agency orientation and clinical training
Compensation
Starting pay is $45.00 per hour, based on experience and qualifications.
