About the Position
Grand Home Health Services is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to provide in-home nursing care and support services for clients receiving home care services.
The LPN works under the supervision of a Registered Nurse (RN) and assists in implementing individualized care plans designed to support the client’s health, safety, and independence at home.
Services are typically provided in private homes and community-based settings.
Position Responsibilities
• Provide direct nursing care to clients in their homes
• Assist with implementation of the client’s individualized care plan
• Monitor and document vital signs and health conditions
• Administer medications as prescribed when authorized
• Assist clients with activities of daily living (ADLs) when needed
• Monitor and report changes in patient condition to supervising RN
• Maintain accurate documentation of care provided
• Educate clients and families regarding health and safety practices
• Follow all infection control and safety procedures
• Maintain compliance with HIPAA and confidentiality requirements
Minimum Qualifications
• Current Pennsylvania Licensed Practical Nurse (LPN) License
• Graduate of an approved Practical Nursing Program
• CPR / BLS certification preferred
• Experience in home care or community health preferred
Required Pennsylvania Clearances
• Pennsylvania State Police Criminal Record Check (Act 34)
• Pennsylvania Child Abuse History Clearance (Act 33)
• FBI Fingerprint Clearance (Act 114) when required
• Act 168 Employment History Review when applicable
• TB screening if required
Required Training
• HIPAA Privacy and Confidentiality Training
• Infection Control Training
• CPR / Basic Life Support (BLS) preferred
• Agency orientation and clinical training as required
Compensation
Starting pay is $33.00 per hour, based on experience and qualifications.
